Ear deformities occur in a number of conditions. Some patients are born with absent (microtia), protruding or drooping ears, due to weak or poorly formed cartilage. Traumatic deformities of the ear also occur due to trauma (e.g. car wreck or dog bite) or torn earlobes. Deformity of the ears may cause social anxiety and may make children vulnerable to teasing. Regardless of the origin of the ear deformity, these ear conditions can be surgically corrected. These procedures do not alter the patient’s hearing, but they may improve appearance and self-confidence.


The surgery begins with an incision behind the ear, in the fold where the ear joins the head. The surgeon Dr. Rahul Goyal remove skin and cartilage or trim and reshape the cartilage. In addition to correcting protrusion, ears may also be reshaped, reduced in size, or made more symmetrical.
The cartilage is then secured in the new position with permanent stitches which will anchor the ear while healing occurs.Typically otoplasty surgery takes about two hours. The soft dressings over the ears will be used for a few weeks as protection, and the patient usually experiences only mild discomfort. is an abnormality of the urethra and penis that is present at birth. This can cause problems with passing urine and also with erections. It can range in severity. Surgical correction is usually very successful


The main problem is that the urethra opens on the underside of the penis instead of at the end of the penis. The opening can be anywhere from just below the normal position (mild) to as far back as the base of the scrotum (severe).


The cost of treatment varies based on a number of factors, including:

  • A hooded appearance of the foreskin. This is because the foreskin does not develop on the underside of the penis.
  • Tightening of the tissues on the underside of the penis (called ‘chordee’). This pulls the penis down and it cannot fully straighten. The further back the urethral opening is, the more severe the chordee tends to be. Mild hypospadias may not have any chordee.

If the hypospadias is mild, with the opening of the urethra just a little down from normal and with no bending of the penis, then no treatment may be needed. However, in most cases an operation is required to correct the hypospadias. This can usually be done in one operation. However, if the hypospadias is more complicated, then two operations may be necessary. The operation is usually done when the child is around 4-18 months old.

The goal of treatment is for urine to be passed in a forward way, for the penis to be straight when erect and for the penis to look as normal as possible. The position of the opening of the urethra is altered. Also, if chordee is present then this is corrected to allow the penis to straighten. The foreskin is usually used during the operation to make the new urethra so it is important that a circumcision is not done before the corrective surgery is performed.

The success of the operation and the ‘normality’ that can be achieved may depend on the severity of the hypospadias.


At your first visit with the plastic surgeon Dr. Rahul Goyal, will discuss with you the details of the surgery, risks, complications, costs, recovery time, and outcome. At this time, your child’s surgeon will answer any questions you may have.


Your child may be irritable following surgery. Your child’s physician may prescribe medications to help with this. Your child may also have to wear padded restraints on his/her elbows to prevent him/her from rubbing at the stitches and surgery site.
Stitches will either dissolve on their own or will be removed in approximately five to seven days. Specific instructions will be given to you regarding how to feed your child after the surgery. The scar will gradually fade, but it will never completely disappear.

During the surgery, and for a short time after surgery, your child will have an intravenous catheter (IV) to provide fluids until he/she is able to drink by mouth. For a day or two, your child will feel mild pain, which can be relieved with a non-aspirin pain medication. A prescription medication may also be given for use at home.
Your child’s upper lip and nose will have stitches where the cleft lip was repaired. It is normal to have swelling, bruising, and blood around these stitches.


This surgery is usually more involved and can cause more discomfort and pain for the child than cleft lip surgery. Your child’s physician may order pain medicine to help with this. As a result of the pain and the location of the surgery, your child may not eat and drink as usual. An intravenous (IV) catheter will be used to help give your child fluids until he/she can drink adequately.
Your child will have stitches on the palate where the cleft was repaired. The stitches will dissolve after several days and they do not have to be taken out by the physician. In some cases, packing will be placed on the palate. Do not take the packing out unless you are told to do so by your child’s physician.

There may be some bloody drainage coming from the nose and mouth that will lessen over the first day.
There will be some swelling at the surgery site, which will diminish substantially in a week.
For two to three days, your child will feel mild pain that can be relieved by a non-aspirin pain medication. A prescription medication may also be given for use at home.
Many infants show signs of nasal congestion after surgery. These signs may include nasal snorting, mouth breathing, and decreased appetite. Your child’s physician may prescribe medication to relieve the nasal congestion.

Your child will be on antibiotics to prevent infection while in the hospital. Your child’s physician may want you to continue this at home.
Your child may be in the hospital for one to three days, depending on your child’s physician’s recommendation.
A small amount of water should be offered after every bottle or meal
to cleanse the incision. You can continue to rinse this area gently with water several times a day, if necessary.


Your child’s physician may allow breastfeeding, bottle-feedings, or cup-feedings after surgery. Your child should be placed on a soft diet for seven to 10 days after surgery. For older infants and children, age-appropriate soft foods may include strained baby foods, popsicles, yogurt, mashed potatoes, and gelatin. Note: your child should not use a straw or pacifier, as both could damage the surgical repair.


Your child can walk or play calmly after surgery. He/she should not run or engage in rough play (i.e., wrestling, climbing) or play with “mouth toys” for one to two weeks after surgery. Your child’s physician will advise you when your child can safely return to regular play.

The abdominal etching procedure usually takes only an hour, and can be performed as an outpatient procedure under either local or general anesthesia. Before surgery, the doctor will outline the patient’s abdominal muscles and use these lines to serve as a guide during surgery. During the procedure, several small incisions are made in the naval or the natural creases of the abdomen, where they are inconspicuous. A cannula, or a flexible tube, is then inserted through the incisions to sculpt the abs by removing excess fat along the lines of the patient’s abdominal muscles. The four or five remaining scars are very small – only about an eighth or a quarter inch in length.


For a few weeks after abdominal etching surgery, patients will need to wear a compression garment, which should only be removed when bathing. You will be given a pre-operative information packet that explains everything you should do and know before your surgery date. Since a variety of procedures can be performed, your individual postoperative instructions may vary. In general, skin grafts require kind of “bolster” dressing to keep them in place for 3-5 days without any movement of the skin graft. The bolster helps the skin graft “stick” to the wound and begin to heal. Smaller operations (scar revisions, Z-plasties) might require only a small, soft dressing afterwards. After scar releases on the hand, your surgeon may place you in a larger dressing that incorporates a plaster splint for support after surgery. In general, follow-up visits are scheduled within 2 weeks of surgery, and there may be sutures to remove at that time.

The classifications for hand deformities can vary. The classifications listed below have been accepted by the American Society for Surgery of the Hand. Classifications may change as more knowledge is obtained regarding each of the conditions. There are seven groups of deformities of the hand that will be discussed, including the following:


This occurs when parts of the body stop developing while the baby is in the womb, causing either a complete absence of a part of the body, such as the hand, or a missing structure, such as part of the arm bone. In the case of the complete missing part, surgery is not indicated. These children may be introduced to prosthetic devices early in their childhood.
Types of this classification include, but are not limited to:

  • Radial clubhand. A radial clubhand is a deformity that involves all of the tissues on the radial side (thumb side) of the forearm and hand. There may be shortening of the bone, a small thumb, or absence of the thumb. Deformities of the wrist are usually operated on around 6 months of age.
  • Ulnar clubhand. An ulnar clubhand is less common than a radial clubhand. This deformity may involve underdevelopment of the ulnar bone (the bone in the forearm on the side of the little finger), or complete absence of the bone.


With this type of deformity, the parts of the hand, either the bones and/or the tissues, fail to separate in the womb. The most common type of this classification is syndactyly. Syndactyly is the most common congenital hand deformity, in which two or more fingers are fused together. There is a familial tendency to develop this deformity. If the fingers are completely fused together, it is considered complete.
There are two types of syndactyly, including the following:

  • Simple syndactyly. This involves fusion between only the tissues of the fingers.
  • Complex syndactyly. This involves fusion between the bones.

Another example of failure of the hand to separate is seen in contractures of the hand. Contractures of the hand may also develop as a result of failure of the cells to differentiate in utero. A contracture is an abnormal pulling forward of the digits of the hand, usually caused by problems with the muscles or skin. One of the common types of this classification includes congenital triggering. Congenital triggering occurs when one of the digits is unable to extend. It is usually seen in the thumb. It may take some time in the child’s development before it is noted that the child can not extend the thumb. Some of these cases improve on their own. Surgery is usually not performed until the second year of life, but preferably before the age of 3.


Duplication of digits is also known as polydactyly. The little finger is the finger that is most often affected.


Underdeveloped fingers or thumbs are associated with many congenital hand deformities. Surgical treatment is not always required to correct these deformities. Underdeveloped fingers may include the following:

  • The digit is small
  • Muscles are missing
  • Bones are underdeveloped or missing
  • There is complete absence of a digit


Overgrowth of digits is also known as macrodactyly, which causes an abnormally large digit. In this situation, the hand and the forearm may also be involved. In this rare condition, all parts of the finger (or thumb) are affected; however, in most cases, only one digit is involved (usually the index finger). Surgical treatment of this condition is complex and the outcomes may be less than desirable. Sometimes, amputation of the enlarged digit is recommended.


This occurs when a tissue band forms around a digit (finger) or limb (arm), causing problems that can affect blood flow and normal growth. Ring constrictions are congenital (present at birth). This condition may be associated with other problems, such as clubfoot, cleft lip, cleft palate, or other craniofacial anomalies. The cause of the ring constrictions is unknown. Some theories suggest that amniotic banding may lead to ring constrictions around a finger or limb.

With this condition, there are four degrees of severity, including the following:
Simple constrictions
Constrictions that cause deformities beyond the constriction
Constrictions that are associated with the fusion of parts of the finger
Constrictions so severe that part of the finger will need to be amputated from the process


Stage I and II bedsores usually heal within several weeks to months with conservative care of the wound and ongoing, appropriate general care. Stage III and IV bedsores are more difficult to treat.


Addressing the many aspects of wound care usually requires a multidisciplinary approach. Members of your care team may include

The first step in treating a bedsore is reducing the pressure that caused it. Strategies include the following:
Repositioning. If you have a pressure sore, you need to be repositioned regularly and placed in correct positions. If you use a wheelchair, try shifting your weight every 15 minutes or so. Ask for help with repositioning every hour. If you’re confined to a bed, change positions every two hours.

If you have enough upper body strength, try repositioning yourself using a device such as a trapeze bar. Caregivers can use bed linens to help lift and reposition you. This can reduce friction and shearing.

  • Using support surfaces. Use a mattress, bed and special cushions that help you lie in an appropriate position, relieve pressure on any sores and protect vulnerable skin. If you are in a wheelchair, use a cushion. Styles include foam, air filled and water filled. Select one that suits your condition, body type and mobility.


Care that helps with healing of the wound includes the following:

  • Cleaning. It’s essential to keep wounds clean to prevent infection. If the
    affected skin is not broken (a stage I wound), gently wash it with water and mild soap and pat dry. Clean open sores with a saltwater (saline) solution each time the dressing is changed.

Applying dressings. A dressing promotes healing by keeping a wound moist, creating a barrier against infection and keeping the surrounding skin dry. Dressing choices include films, gauzes, gels, foams and treated coverings. A combination of dressings may be used.

Your doctor selects a dressing based on a number of factors, such as the size and severity of the wound, the amount of discharge, and the ease of placing and removing the dressing.


To heal properly, wounds need to be free of damaged, dead or infected tissue. Removing this tissue (debridement) is accomplished with a number of methods, depending on the severity of the wound, your overall condition and the treatment goals.

  • Surgical debridement involves cutting away dead tissue.
  • Mechanical debridement loosens and removes wound debris. This may be done with a pressurized irrigation device, low-frequency mist ultrasound or specialized dressings.
  • Autolytic debridementenhances the body’s natural process of using enzymes to break down dead tissue.This method may be used on smaller, uninfected wounds and involves special dressings to keep the wound moist and clean.
  • Enzymatic debridement involves applying chemical enzymes and appropriate dressings to break down dead tissue


diabetic foot ulcer is an open sore or wound that most commonly occurs on the bottom of the foot in approximately 15 percent of patients with diabetes. Of those who develop a foot ulcer, six percent will be hospitalized due to infection or other ulcer-related complication.

Anyone who has diabetes can develop a foot ulcer. Native Americans, African Americans, Hispanics and older men are more likely to develop ulcers. People who use insulin are at a higher risk of developing a foot ulcer, as are patients with diabetes-related kidney, eye, and heart disease. Being overweight and using alcohol and tobacco also play a role in the development of foot ulcers.


A majority of noninfected foot ulcers are treated without surgery; however, when this fails, surgical management may be appropriate. Examples of surgical care to remove pressure on the affected area include shaving or excision of bone(s) and the correction of various deformities, such as hammertoes, bunions, or bony “bumps.”

A skin graft is a piece of healthy skin removed from one area of your body to repair damaged or missing skin somewhere else on your body. This skin does not have its own source of blood flow.
A skin flap is healthy skin and tissue that is partly detached and moved to cover a nearby wound.

  • A skin flap may contain skin and fat, or skin, fat, and muscle.
  • Often, a skin flap is still attached to its original site at one end and remains connected to a blood vessel.
  • Sometimes a flap is moved to a new site and the blood vessel is surgically reconnected. This is called a free flap.

The area from where skin is taken is called the donor site. After surgery, you will have two wounds, the graft or flap itself and the donor site.
Learning how to care for skin flaps and grafts can help them heal more quickly and reduce scarring.

Skin grafts are used to help more serious wounds heal, including:

  • Wounds that are too big to heal on their own
  • Burns
  • Skin loss from a serious skin infection
  • Surgery for skin cancer
  • Venous ulcers, pressure ulcers, or diabetic ulcers that do not heal
  • After mastectomy or amputation

Donor sites for grafts and flaps are chosen based on:

  • How closely the skin matches the area of the wound
  • How visible the scar will be from the donor site
  • How close the donor site is to the wound

Often the donor site may be more painful after surgery than the wound due to newly exposed nerve endings.

You will need to care for both the flap or graft site as well as the donor site. When you come home after surgery, you will have a dressing on your wounds. The dressing does several things, including

  • Protect your wound from germs and reduce the risk of infection
  • Protect the area as it heals
  • Soak up any fluids that leak from your wound

Often the donor site may be more painful after surgery than the wound due to newly exposed nerve endings.

To care for the graft or flap site:

  • You may need to rest for several days after surgery as your wound heals.
  • The type of dressing you have depends on the type of wound and where it is.
  • Keep the dressing and area around it clean and free from dirt or sweat.
  • Don’t let the dressing get wet.
  • Do not touch the dressing. Leave it in place for as long as your doctor recommends (about 4 to 7 days).
  • Take any medicines or pain relievers as directed.
  • If possible, try to elevate the wound so it’s above your heart. This helps reduce swelling. You may need to do this while sitting or lying down. You can use pillows to prop up the area.
  • If your doctor says it’s OK, you may use an ice pack on the bandage to help with swelling. Ask how often you should apply the ice pack. Be sure to keep the bandage dry.
  • Avoid any movement that might stretch or injure the flap or graft. Avoid hitting or bumping the area.
  • If you have a vacuum dressing, you may have a tube attached to the dressing. If the tube falls off, tell your doctor.
  • You will probably see your doctor to have your dressing changed in 4 to 7 days. You may need to have the dressing to your flap or graft site changed by your doctor a couple times over 2 to 3 weeks.
  • As the site heals, you may be able to care for it at home. Your doctor will show you how to care for your wound and apply dressings.
  • The site may become itchy as it heals


The brachial plexus (brachial means arm and plexus mean communication or meeting point) refers to the nerves that exit the cervical spine and pass down to the shoulder and arm. Five major nerves comprise the brachial plexus: C5, C6, C7, C8, and T1 (C refers to cervical [or neck] and T refers to thorax [or chest]). These nerve pass under the skin in the neck and axilla, where they are vulnerable to injury. When the neck and arm are forced away from each other during trauma (e.g., car accidents, motorcycle accidents, falls) the brachial plexus nerves can be stretched or torn apart. If the force is severe, these nerves can even be pulled away from the spinal cord where they originate. Damage to these nerves causes pain, numbness, and weakness in the shoulder, arm, and hand. The pain can be quite severe, and is often described at burning, pins and needles, or crushing. In general, the C5 nerve controls the rotator cuff muscles and shoulder function, C6 controls flexing the arm at the elbow, C7 partially controls the triceps and wrist flexion, and C8/T1 controls hand movements. Several patterns of injury occur, the most common is referred to as an Erb’s palsy. This is when C5 and C6 are predominantly affected. These patients are unable to lift their arm or flex at the elbow. Severe atrophy can occur in the shoulder muscles. Another pattern of injury is when C8/T1 is mostly damaged.
